The Questionnaires
The 2008 Arizona Health Survey iscomprised of a screener and surveys for three age groups:
- Adults (18 years of age and older), including proxy interviews for frail elderly adults;
- Adolescents (ages 12 to 17), who were interviewed directly with permission of a parent or guardian; and,
- Children (birth to 12 years of age), for whom Arizona Health Survey interviews the adult in the household who is most knowledgeable about the child’s health.
Topics and questionnaires were developed through a collaborative process designed to balance the information needs of community-based organizations, policymakers and researchers with consideration for the burden on respondents, effect on response rates and available funding. Final questionnaire design resulted in adult interviews that averaged 30 minutes, child interviews that averaged 15 minutes, and adolescent interviews that averaged 12 minutes.
